New Patient Registration

You can apply to register in person by visiting either our Wellingborough or Earls Barton surgeries where the duty receptionist will be able to assist you.

For your convenience, the forms can be downloaded below. 

You can bring the completed forms to the surgery along with Passport or Photo ID, utility bill, and birth certificate for children. 

You will be required to provide evidence of entitlement to NHS services, proof of identity and residency. 

The receptionist will also arrange for you to have a new patient registration routine medical check appointment.

Online Registration
Applying to register as a patient at the Abbey Medical Practice via the online method has temporarily been suspended due to changes at the PCT level. This will be resumed shortly.
