Online Patient Registration

Applying to register as a patient at the Abbey Medical Practice. 

If you wish to apply to become a registered patient, please read these notes and then use the link below to submit your application (please note that a separate application is required for each family member).

1. Applications will only be considered if you reside within the Practice area, which is defined as follows:  Wellingborough(NN8), Earls Barton(NN6), Wilby(NN29), Great Doddington (NN29), Mears Ashby(NN6), Ecton (NN6), Grendon (NN7), Hardwick(NN9), Little Harrowden(NN14), Great Harrowden(NN14),Orlingbury (NN14), Little Irchester(NN8), Irchester (NN8), Finedon (NN10) and Wollaston (NN29).
2. Please answer every question on the application form as fully as possible.
3. When you have submitted the application form, click on the ‘complete GMS Form 1’ button and answer all questions giving as much detail as possible.  Incomplete forms will inevitably delay the process and could, in extreme circumstances, invalidate your application.
4. Submission of an application DOES NOT infer acceptance.
5. If you prefer, you can apply to register in person by visiting either our Wellingborough or Earls Barton surgeries where the duty receptionist will be able to assist you.
