Recording of your Country of Origin

All patients have different healthcare needs and we wish to ensure that the services we provide meet the needs of all our patients. To help us achieve this, we are asking all patients over 16 years of age to let us know:

• a their ethnic origin
  
• b their preferred spoken language
• c whether they are, or have, a carer

You may have already completed a form giving us these details. If you have not done so please ask the receptionist for a questionnaire. This information is very important in helping us plan our services for you appropriately.
