Depression, Anxiety & Stress

Healthy Mind Practitioner

The Healthy Mind Practitioner (HMP) is a qualified mental health nurse within Abbey Medical Practice whose role is one of advice and signposting primarily for patients experiencing difficulty with their mental health. Secondary to this the HMP can liaise with the GP’s relating to starting and reviewing mental health medication.

Advice will be around lifestyle choices and changes that can be made to benefit a person’s mental health and often based around a person’s day to day routines. The educational focus can be around patterns of eating, sleeping, exercise and self-care as examples of changes that can positively impact someone’s mental health.

The HMP can also signpost to other support services depending on need but regular examples are to the talking therapy services at Changing Minds/IAPT and Mind who provide support for mild to moderate mood and anxiety issues. For more complex cases referrals to secondary care are considered.

The HMP approach is collaborative and patient centred ensuring the patient can make fully informed and positive choices relating to their mental health needs.
