New Patient Registration

Thank you for expressing an interesting in becoming a patient at Abbey Medical Practice. We welcome new patients who reside in Wellingborough and surrounding villages. If you are not within our catchment area, we will not be able to register you.

You will need to complete our online form. The form should take 10 minutes to undertake and will require you to fully complete the questionnaire to enable your registration to be processed. Documents can be uploaded electronically. If you are registering your family, kindly ensure that parents, guardians, carers undertake their registration first. For new parents, please use the same form and upload the birth certificate for your baby.

After submitting the form, this will take no longer than 2 working days to process and you will receive a text to confirm that you have been registered at the Practice. NHS England will forward your medical records to the Practice, however, this may take up to 2 months.

Please ensure you have a sufficient supply of medication whilst the new patient registration process is being completed.

If you have a long term condition, we will contact you when your review is due. If you wish to have a new patient check and do not have any current health conditions, please contact us and we will be happy to arrange.

Please visit Wellingborough or Earls Barton to speak to a member of the Patient Services Team who will be happy to discuss the registration process with you. Forms can be collected and completed on the day with proof of identification and confirmation of address.

If you are taking regular medication, please provide us with your repeat slip. This will enable our Clinical Pharmacist to process your medication. Please ensure you have a sufficient supply of medication whilst the new patient registration process is being completed.

After completing the form, this will take no longer than 2 working days to process and you will receive a text to confirm that you have been registered at the Practice. NHS England will forward your medical records to the Practice, however, this may take up to 2 months.
