Hello, my name is Chelsea and I am the Practice Care Coordinator. My role is all about reaching out to our patients who need support with their non clinical needs. Since joining the Practice I have worked with many patients who have utilised local services to provide them with emotional, physical and practical support. The impact for these patients has been tremendous and really life changing. It can often be the little things that make the biggest difference and it is a real privilege to be part of this change.
Here at Abbey Medical Practice, we believe that unpaid carers and the challenges of caring should be recognised in all areas of life. Caring should be valued and respected by everyone in our society, and carers should have access to the information and support they need, where and when they need it.
This year’s carers week theme is “Making Caring Valued, Visible and Supported”. There are so many aspects as a carer and this is being celebrated and talked about in great detail this year. Alongside the theme for this year, the focus also touches on “Health and Social Care Issues” and “Work and Employment”.
