Medical Report Request

Under the GDPR 2018 regulations, individuals have the right to apply for access to their health records free of charge. If  you do not wish to use the information provided via our online access and would like to request or have a requirement for a medical report to be sent to a 3rd Party provider please use this Subject Access Request form.  We are unable to submit any data to any third party without your signed consent.

The Practice will review the request and provide a copy of the data within 21 days.
