Bowel Cancer
Bowel screening aims to find cancer early or to find changes in your bowel that could lead to cancer.
If you are aged between 58 and 74 years you will be issued with a screening kit. This test is called a FIT (Faecal Immunochemical Test) and can be conducted at home. The instructions are very clear on how you obtain a sample of poo and how you return this to the laboratory for testing. Checks are undertaken to detect any traces of blood in the poo which can be caused by other medical conditions and doesn’t necessarily mean you have cancer. But if it is cancer, finding it at an early stage means treatment is more likely to work.
Dr Az Ali and Carrie Glenn have made a short video for our patients which provides information as to why it is important to complete this test.
Please visit YouTube - Why it is important to attend your bowel screening to view.
If you wish to organise your test, please contact 0800 707 6060 quoting your NHS number.
For further information please visit Cancer Research - Testing for blood in your poo using the FIT test.
If you haven’t attended your screening, a member of our Patient Services Team will contact you to encourage to have this test.

Healthy Mind Practitioner
The Healthy Mind Practitioner (HMP) is a qualified mental health nurse within Abbey Medical Practice whose role is one of advice and signposting primarily for patients experiencing difficulty with their mental health. Secondary to this the HMP can liaise with the GP’s relating to starting and reviewing mental health medication.
Advice will be around lifestyle choices and changes that can be made to benefit a person’s mental health and often based around a person’s day to day routines. The educational focus can be around patterns of eating, sleeping, exercise and self-care as examples of changes that can positively impact someone’s mental health.
The HMP can also signpost to other support services depending on need but regular examples are to the talking therapy services at Changing Minds/IAPT and Mind who provide support for mild to moderate mood and anxiety issues. For more complex cases referrals to secondary care are considered.
The HMP approach is collaborative and patient centred ensuring the patient can make fully informed and positive choices relating to their mental health needs.

Pre-Diabetes
Pre diabetes is a diagnosis given when someone has a raised blood sugar and is at risk in the future of developing Diabetes. In the last 10 years the number of patients with this condition has increased by almost 100% and if, as a nation, we do not take steps to try and reverse this trend, not only will individuals become unwell, they are at risk of going into hospital, living with ill health and even dying younger.
But this can be reversed if it is diagnosed and people are willing to change their lifestyle.
Diabetes UK report that up to 50% of people can reverse pre- diabetes if they can reduced the sugar in their diet, lose weight and increase activity.
If you have been diagnosed with this condition I urge you to take up the offer of education to try and manage this and make positive changes to your health that will enable to you to not just live longer but in good health and with enjoyment.

Gambling
East Midlands Gambling Harm Service
Many people take part in gambling of one form or another. For some, it can be an enjoyable activity. But other people find that gambling harms their physical and mental health, relationships and performance at work. It can leave them in serious debt too.
East Midlands Gambling Harms Service provide specialist therapies and treatment to people in the East Midlands affected by gambling addiction and gambling problems. They are an NHS team of mental health professionals. With the team they have “experts by experience” – people have recovered from a gambling addiction themselves.
Through group workshops and therapy sessions, they can help people to learn how to control the urge to gamble and adopt more realistic attitudes about the prospects of winning. Also give people tools to build an alternative, more meaningful lifestyle, free from gambling.

Hormone Replacement Therapy (HRT)
HRT
HRT is a treatment to relieve symptoms of the menopause. It replaces hormones that are at a lower level as you approach the menopause. There are many types of HRT for women and it is important that you have a discussion with an Advanced Nurse Practitioner to understand which type of HRT would be suitable for you.
For further information visit:
HRT Review
We know that most women who take HRT lead busy lives and struggle to find time to attend appointments. So, we are trying a new way of conducting our HRT reviews that allows us to get the information we need to safely prescribe you this medication whilst minimising the impact on your time. We are asking you to complete and return a questionnaire to provide us with this information and this can be completed from the comfort of your own home and at a time convenient for you.
Please watch the following video: BMS TV - British Menopause Society (thebms.org.uk)
What do I need to do?
The questionnaire asks all the necessary questions that we require that will enable us to continue to prescribe your HRT. It is important that you answer every question and it is essential that you provide a blood pressure reading otherwise we will not be able to safely review your medication which will result in a delay in you getting your prescription.
If you are genuinely unable to answer these questions then please contact the Practice and explain you are unable to complete the questionnaire and require face to face appointment with an Advanced Nurse Practitioner.
Please don’t worry if you have something you wish to discuss. Answer as many questions and complete the additional section where you can ask any questions. Submit the questionnaire as above. You will be asked to make an appointment with our Advanced Clinical Practitioner to discuss any of your concerns.
How do I check my blood pressure?
We have found that most people have their own blood pressure monitoring machine or know someone who does. If this is not the case, most high street chemists will happily check your blood pressure as a walk in appointment for free.
What happens once I have submitted my questionnaire?
Once submitted, a clinician will review the information provided and make a decision as to whether it is safe to continue to prescribe HRT for another year. Once completed, you will then receive a text message from us to confirm your prescription is ready for collection.
Simple as that!!
If the clinician feels it is not safe for you to continue your HRT, you will be asked to attend an appointment with an Advanced Nurse Practitioner to discuss further.

NHS Digital Weight Management Programme
The adult population living with overweight in England is 25.6 million (62%), of which 11.4 million adults (25%) are living with obesity.
Obesity is a serious health concern that increases the risk of many other health conditions, including Type 2 Diabetes, cardiovascular disease, joint problems, mental health problems and some cancers. There is also evidence to suggest that people living with obesity are at a higher risk of more severe outcome associated with COVID-19.
The NHS Digital Weight Management Programme offers online support to people living with obesity who also have diabetes or hypertension, or both, over 18 years of age, to help them to manage their weight. There are different ways in which you can receive support to change behavioural and lifestyle changes to reach your goals.
If you have often thought “I must do something about losing weight”, now is the time. The support is here for you, it’s just taking the first step to engage – you can do it!
